What is a pilonidal sinus?

A pilonidal sinus is a tract or cavity under the skin of the natal cleft, at the top of the buttocks. Loose hairs are driven into the skin and set up a chronic foreign-body reaction, producing a cavity that becomes infected. It is most common in young adults, more so in men, and in people who sit for long periods or are hairy in that area.

Laser treatment (SiLaT / laser pilonidoplasty) works by cleaning the cavity and its tracts of hair and debris, then passing a radial laser fiber along them. The energy destroys the lining and the cavity collapses and closes. The pits are dealt with through very small openings, so there is no large midline wound and no months of packing.
